It started as one word on a letter from the clinic: methylphenidate. Then the prescription said Concerta XL. The pharmacy label said something else again. Someone online mentioned Ritalin, someone else said "XL" and "MR" are not the same thing, and now there is a number on the box, 18, 36 or 54, that nobody has fully explained.
If that sounds familiar, you are not alone, and you are not missing something obvious. This medicine has more names, strengths and versions than most, and the information is spread across leaflets, forums and pages that assume you already know the basics.
This guide puts it in one place. It explains what Concerta XL is, how the tablet works through the day, what each strength means (including 54mg), the side effects listed in the official product information, and the rules that apply because it is a controlled drug. It also covers the practical part: getting your prescription dispensed and delivered without the runaround.
Everything clinical here comes from the official UK product information or national guidance, and it has been reviewed by a GPhC-registered pharmacist. It does not replace your prescriber. It should make your next conversation with them a lot easier.
what is Concerta XL? Concerta XL is a UK brand of methylphenidate hydrochloride in a once-daily, prolonged-release tablet, used to treat ADHD in children aged 6 and over and in adults. The maximum dose is 54mg a day for children and 72mg a day for adults. It is a Schedule 2 controlled drug in the UK.

Concerta XL is a brand of methylphenidate, a central nervous system stimulant used to treat attention deficit hyperactivity disorder (ADHD). It is only available on prescription, and the product information states treatment should be started under the supervision of a specialist in ADHD.
Yes and no. Concerta XL contains methylphenidate, the same active ingredient as Ritalin and generic methylphenidate tablets. What makes it different is the tablet design. Standard methylphenidate tablets are immediate-release and usually need to be taken two or three times a day. Concerta XL is a prolonged-release tablet built to release the medicine across the day, so it is taken once in the morning.
Not necessarily. Several modified-release methylphenidate products are available in the UK, and they release the medicine at different rates through the day. Because of this, they are usually prescribed by brand, and one should not be swapped for another without your prescriber's agreement. If your pharmacy label shows a different brand from the one you expected, ask your pharmacist before taking it.
Methylphenidate is thought to work by blocking the reuptake of two brain chemicals, noradrenaline and dopamine, and increasing their release. These chemicals play a part in attention, focus and impulse control.
The Concerta XL tablet uses an osmotic release system:
Because the medicine is released from inside the shell, the empty tablet shell passes out of the body. It is normal to occasionally notice something that looks like a tablet in your stool. The medicine has still been absorbed.
According to the UK product information, Concerta XL is licensed as part of a wider ADHD treatment programme for:
Treatment should be started under the supervision of a specialist in ADHD.
NICE guideline NG87 recommends methylphenidate as the first-line ADHD medicine for children aged 5 and over, and lists methylphenidate and lisdexamfetamine as first-line options for adults.
Concerta XL is not licensed for children under 6.

Your prescriber decides your dose. The UK product information sets out the following.
If you already take immediate-release methylphenidate three times a day, your prescriber may switch you to Concerta XL at a dose that roughly matches your total daily amount. The product information gives these guide conversions:
| Previous immediate-release dose | Concerta XL dose |
|---|---|
| 5mg three times a day | 18mg once a day |
| 10mg three times a day | 36mg once a day |
| 15mg three times a day | 54mg once a day |
| 20mg three times a day (adults) | 72mg once a day |
The product information advises that if there is no improvement after appropriate dose adjustments over one month, treatment should be stopped. It also recommends that prescribers stop treatment at least once a year to check whether it is still needed, often as a planned break.
Do not change your dose yourself. If you feel your dose is too strong, not strong enough, or wearing off too early, tell your prescriber.
54mg is the strongest single Concerta XL tablet, and one of the most searched-for strengths.
If you have been prescribed 54mg and have questions about side effects or how long it lasts, speak to your prescriber or pharmacist.
Because the tablet must be swallowed whole, Concerta XL is not suitable for people who cannot swallow tablets. The product information also advises against using it in people with severe narrowing of the gut. If swallowing is a problem, tell your prescriber.
Store your tablets securely and out of reach of children. Because it is a controlled drug, it must never be shared.
Concerta XL starts working on the day you take it. The outer coating releases some medicine within the first hour or two, and the rest is released gradually. In clinical studies, the effects were maintained until 12 hours after dosing.
How long it feels like it lasts varies between individuals. Some people notice it wearing off in the late afternoon. If this affects homework, work or evenings, mention it at your review rather than taking extra.
Like all medicines, Concerta XL can cause side effects, although not everybody gets them. Many are most noticeable at the start or after a dose increase.
The Concerta XL product information lists:
Contact a doctor straight away, or call 999, if you or your child has:
This is not a full list. Read the Patient Information Leaflet in your pack for every known side effect. You can report suspected side effects through the MHRA Yellow Card scheme.
These are general tips. Speak to your prescriber or pharmacist if a side effect is bothering you.
According to the UK product information, Concerta XL must not be used by people who:
Your prescriber will also take extra care if you have a history of seizures, tics or Tourette's syndrome, substance misuse, or a family history of sudden death or heart rhythm problems. Tell them about all of these before you start.
Always tell your prescriber and pharmacist about every medicine you take. Key interactions listed in the product information include:
It can. The leaflet states methylphenidate may give a positive result on drug tests, including tests used in sport. If you are tested, tell the tester you take a prescribed medicine.
Before you start, and throughout treatment, your prescriber will check:
These checks are a normal part of safe treatment.
The product information states Concerta XL is not recommended in pregnancy unless a clinical decision is made that delaying treatment poses a greater risk. Methylphenidate passes into breast milk, so a decision about breastfeeding should be made with your prescriber. If you are pregnant, planning a pregnancy or breastfeeding, speak to your prescriber.
Concerta XL can cause dizziness, drowsiness and blurred vision. Do not drive until you know how it affects you. In the UK, it is an offence to drive if a medicine impairs your driving, but there is a legal defence if you are taking a prescribed medicine as directed and it is not affecting your driving.
| Medicine | Type | Controlled drug? | How it is taken |
|---|---|---|---|
| Concerta XL (methylphenidate) | Stimulant, prolonged-release | Yes, Schedule 2 | Once daily, morning |
| Immediate-release methylphenidate (e.g. Ritalin) | Stimulant | Yes, Schedule 2 | Usually two or three times a day |
| Other modified-release methylphenidate brands | Stimulant | Yes, Schedule 2 | Once daily, release profile varies by brand |
| Elvanse (lisdexamfetamine) | Stimulant (prodrug) | Yes, Schedule 2 | Once daily, morning |
| Dexamfetamine (e.g. Amfexa) | Stimulant | Yes, Schedule 2 | Usually more than once a day |
| Atomoxetine | Non-stimulant | No | Daily |
| Guanfacine (Intuniv) | Non-stimulant | No | Once daily |
Concerta XL vs Ritalin: same active ingredient, different release. Ritalin (immediate-release) acts faster and wears off sooner, so it needs more than one dose a day. Concerta XL is designed to cover the day with one morning dose.
Concerta XL vs Elvanse: different medicines. For children, methylphenidate is usually tried first, and Elvanse may be considered if methylphenidate has not helped enough. Read our Elvanse (lisdexamfetamine) guide for more.
Methylphenidate is a Schedule 2 controlled drug under the Misuse of Drugs Regulations 2001. The product information notes long-term misuse can lead to tolerance and psychological dependence, so extra legal rules apply to how it is prescribed and supplied.
In practice:
These are legal requirements, not the pharmacy being awkward. A well-organised pharmacy will explain them and help you plan around them.
On the NHS: you pay the standard prescription charge in England, unless you are exempt. Prescriptions are free in Scotland, Wales and Northern Ireland, and for under-16s.
Privately: with a private prescription, you pay for the medicine itself. At Pharmazon HomeCare:
| Medicine | Price |
|---|---|
| Concerta XL tablets (methylphenidate) | From £43.11 per pack |
| Methylphenidate XL tablets (prolonged-release) | From £34.75 per pack |
| Methylphenidate tablets (immediate-release) | From £15.09 per pack |
Prices vary by strength and pack size. A valid prescription is required. Your prescriber decides which brand you take, and modified-release brands should not be swapped without their agreement.
Private prescriber or clinic fees are separate from the cost of the medicine.

Concerta XL is used to treat ADHD in children aged 6 and over and in adults, as part of a wider treatment programme. It contains methylphenidate in a once-daily, prolonged-release tablet. Pharmazon HomeCare dispenses valid Concerta XL prescriptions from GPs, psychiatrists and private clinics.
They contain the same active ingredient, methylphenidate, but release it differently. Ritalin is immediate-release and is usually taken more than once a day. Concerta XL is prolonged-release and is taken once in the morning. Pharmazon HomeCare dispenses the exact product on your prescription.
In clinical studies, its effects were maintained until 12 hours after a morning dose. This varies between individuals, so speak to your prescriber if it wears off early.
The maximum is 54mg a day for children and 72mg a day for adults. Your prescriber decides your dose and increases it gradually, usually in 18mg steps.
54mg is the highest single tablet strength and the maximum daily dose for children. Adults can be prescribed up to 72mg a day. It roughly matches 15mg of immediate-release methylphenidate three times a day. Our pharmacists can answer questions about your strength when we dispense your prescription.
At Pharmazon HomeCare, Concerta XL tablets start from £43.11 per pack. Prices depend on strength and pack size, and private prescriber fees are separate. Get in touch with our pharmacy team for a current price.
The most common side effects are headache, difficulty sleeping, nervousness, reduced appetite and cold-like symptoms. Seek urgent help for chest pain, an irregular heartbeat, suicidal thoughts, hallucinations or a seizure.
No. Concerta XL must be swallowed whole with water. Chewing, splitting or crushing it damages the release system. If swallowing tablets is difficult, speak to your prescriber.
That is the empty tablet shell. The medicine is released from inside, and the shell passes out of the body. This is normal and does not mean the medicine has not worked.
No. The Patient Information Leaflet advises not drinking alcohol while taking Concerta XL, because it may make side effects worse.
Do not take a double dose. Wait until your next dose is due and take it as normal in the morning.
